Person-centered care is used, misused and incorrectly defined, although the concept is widely used in the UK and catching on in the USA. Person-centered care is a viable concept for the day-to-day management of dementia patients in institutional settings and at home. This presentation will define the key concepts of person-centered care, give a fresh definition of the concept, explore the key components and give specific examples of how nurses can implement person centered programs and interventions in their practice. The focus of the presentation will be on providing clear implementation guidelines and identifying best practices/research findings to model. At the completion of the presentation, the attendee will be able to state a clear definition of person-centered care, list the key concepts, and write an implementation plan for their own practice. A comprehensive reference list will be provided to attendees.
